Description

Tadalafil 22mg troches are compounded sublingual medications designed for the management of erectile dysfunction (ED), benign prostatic hyperplasia (BPH), and pulmonary arterial hypertension (PAH). Tadalafil is a phosphodiesterase type 5 (PDE5) inhibitor that enhances smooth muscle relaxation and vasodilation by increasing cyclic guanosine monophosphate (cGMP) levels. 

The troche is administered by placing it in the mouth and allowing it to dissolve completely, ensuring effective sublingual absorption and bypassing first-pass metabolism. Caution is advised in patients with underlying cardiovascular conditions, and concurrent use with nitrates or alpha-blockers is contraindicated due to the risk of severe hypotension.

Potential adverse effects include priapism, visual disturbances, and dizziness. Tadalafil may interact with CYP3A4 inhibitors, altering plasma levels and increasing the risk of side effects. The use of tadalafil in conditions such as Raynaud's phenomenon and female sexual arousal disorder is off-label and requires further clinical evidence.
